Registration Information
Registration for Spring 2010.
If you are a player who is being added to a WSYSC team roster for Spring 10, you need to register online by clicking the Online Registration Form below. Online Registration Form
Also, Players need to bring the following to your coach.
- Zero Tolerance form – signed by player and parents
- MYSA Membership form – filled out completely
- Copy of player's Birth certificate
- Check for $60 (spring cost only) made out to "WSYSC"
- One inch by one inch recent color head shot of the player (U11 – U14)
The $60 cost is for Spring Season only. (FYI, The yearly cost per player is $100.)
Any New Coaches: You need to provide/update the following forms
- Team Roster – Roster will be generated by the WSYSC Board, you will sign after confirming players
- Zero Tolerance Form – signed by new coach
- MYSA Membership Form – filled out completely by coach
- CORI Form filled out on-line by coach
- One inch by one inch recent color head shot of the coach (U9 – U14)
- Coach signature on the "Coach Pass Card" (U9 – U14) (see Club Registrar for Pass Card)
Existing Coaches: it is your responsibility to provide the following:
- Updated Team Roster – signed, circle your division requested
- New player forms, fees, pass cards
- All pass cards from Fall Season
- Place uniform orders if needed
